If you believe EVHS has failed to meet its obligations as explained in the "Notice of Privacy Practices" or the legal obligations under State or Federal Law, you may contact the HIPAA Privacy Officer at 215-600-3795 Option 6 regarding your complaint.  You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services within 180 days of the date you know or should know of the act that is the subject of your complaint.  Your complaint must be filed in writing, either electronically or on paper.
